Here is the short version of the difference between Eastern and Western North Carolina-style barbecue.

In the east, barbecue is made from the whole hog. Eastern sauce uses no tomato extracts, and no added sugar. It is primarily vinegar, water, salt, black pepper, red pepper, and cayenne pepper.

Western-style, also known as Lexington-style uses only the shoulder of the hog. Western sauce is basically the Eastern sauce with ketchup, perhaps some Worcestershire, and a little brown sugar added.
